• Licensure Verification - Each healthcare professional is required to show proof of active licensure, and each license is verified and documented.
  
  
• References - All healthcare professionals must have a minimum of two clinical references from previous supervisors.
  
  
• Completed Skills Checklist - All healthcare professionals must complete a detailed checklist regarding their skills and experience.
  
  
• Drug Screening - All MDI Medical employees must undergo and pass a ten-panel drug screen prior to beginning an assignment.
  
  
• Criminal Background Check - A criminal background check covering the last 7 years is completed prior to starting an assignment. MDI Medical will not employ any therapist with felony or misdemeanor convictions for crimes of violence, perjury, theft, or moral turpitude or any conviction related to client services or patient care.
  
  
• Health Information/Documentation - All healthcare professionals must submit a completed physical examination form stating they are in good health and have up-to-date immunizations and PPD.

MDI Medical's Continued Compliance Policy ensures that on going employment requirements are maintained. Our Credentialing Manager monitors expirations for the following credentials and generates monthly reports to identify expirations in advance of the expiration date. When employee credentials are updated, MDI Medical updates the employee's file and informs the client. Currently, we monitor the following:

• Annual Drug Screen and Criminal Background Check
• Annual Physical Form
• Annual Skills Checklist
• Current Immunizations and PPD
• Active License in Good Standing
• CPR/BLS Certificate renewal
